For the smoothest results (at the expense of rendering time), the trick is to apply a separate shader to the glass and set base shade to 0.1. Also set the AA to 32.

If you leave base shade at 1.0, wether AA is 8 or 64, it won't make difference and the quality will not be so good.

The other thing is adding fresnel or reflection doubles rendering time but makes it look nicer. You still get nice results without it.
